Position Summary:

This position requires an Associate Degree in Health Info Management, and RHIT Certification.


Assist in the coordination of HIM department activities and initiatives. Provide training for various HIM operational and analytical tasks and projects to include prepping, scanning, chart analysis and release of information. Organize and track correspondence with providers regarding incomplete documentation needed for coding and billing; resolve documentation problems in the EMR for providers; Research and merge duplicate medical record number and encounter issues; Change and update patient demographic information. Assist in coordinating offsite storage sending and retrieval for departments. Audit prepped and scanned documentation to ensure it meets department productivity and quality standards. Provide customer service to patients and third party requesters when processing requests for medical records. Process requests for information ensuring all disclosures follow HIPAA guidelines and minimum necessary requirements; Electronically process and track RAC, ADR and other third party payer requests. Analyze various documentation criteria to report compliance statistics and data. Perform transcription tasks and audits of transcribed documents to ensure transcription is meeting department standards. Perform other related duties as assigned. Ensure adherence to Hospitals and departmental policies and procedures. No patient care assignment.
Accountability:
  • REVIEWS – Audit medical records to ensure they meet TJC accreditations and CMS requirements. Provide analysis for reporting of documentation trends. Provide feedback to departments and providers when documents are missing or not authenticated per medical staff rules, regulations, and department guidelines

  • TIMELINESS– Ensure all requests for information are completed in accordance to federal, state and regulatory timeliness guidelines. Audit documentation processing to ensure it is completed within established timeliness standards

  • QUALITY- Direct, implement and make continuous improvement to HIM functions. Monitor and report identified issues with document preparation, scanning, quality check, or validation by technical and clerical staff

  • EDUCATION- Provide training and guidance to staff for various HIM duties to include scanning, quality checking, validation, and chart analysis; Maintain and update educational tools and instructions for current and new staff

  • PATIENT INFORMTATION REQUESTS- Electronically and manually process all types of requests for information to be released in accordance to HIPAA standard; RAC, ADR, HEDIS, etc.

  • CONFIDENTIALITY- Maintain medical record confidentiality and legal requirements

  • RECORDS - Create and maintain daily records of work performed

  • DEVELOPMENT- Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meeting and workshops

  • POLICIES AND PROCEDURES- Ensure adherence to institutional policies and procedures for administration and maintenance of medical records
